Add friction only to risky actions

Require approval for large refunds, post-close voids, payout changes, exports, and permission edits. Use a second factor for administrators and payment settings. Log the actor, time, device, old value, new value, and reason.

Review privileged accounts monthly and remove leavers immediately. Test the recovery process before an emergency; security that locks the business out is not resilient.
